Description:	Need for someone to create a Quote page for Zen Cart will need the following fields. Name (will need to be required field) Email (will need to be required field) Phone (will need to be optional field) Color (will need to be required field) Size (will need to be required field) Quantity (will need to be required field) Upload box (will need to be required field also will need to have a browse option so they can browse there computer for the file they want to upload) Time frame (will need to be required field) Comments box (will need to be optional field) Will need for the required fields to kick back an error if they have not been completed. Will need to have the ability to upload a file (zip, rar & all image formats no videos or exe's). Will also need to have comments box. Once the form is completed it will use the servers send mail so all this information plus the file are emailed to me. Will also need for it to send them a confirmation email telling them we will get review there request and will contact them within 48 hours. Also if possible to have the file they upload to also be saved in a folder on the server something like *quotes*
