Description

I need to get an online employment application done quickly and accurately using the NMS form handler or a php solution. Results would be emailed to recipient in plain text. There are approximately 50 fields (some common) that require text input by the user.

Ideally I'd like it matched to the client's brand, but at this point I need the done - pretty can come later.
