Description: We need a form with a group of select boxes to populate based on user selection, the form needs to be dynamic with mysql or xml.

you can see a working front-end example of how the form should function:

The user chooses a year:
Year
That would populate
Make
Then that would populate
Model

The backend would be a simple password protected, insert update delete
with a mysql database or xml.
