Description:	I have a small system that provides a directory of synagogues. One feature, we apparently left out or lost was that the administrator needs to be able to key in data (from publicly available information) to add new synagogues to the list. There is a currently a form that allows "self-registration", but we want the administrator to pre-register. No changes to the database should be required, just a new PHP form that is modeled after an existing one. I would prefer you do the work directly on my system. Also would need a new menu-item on the administrators menu.
